Endoscopic Submucosal Dissection (ESD) Introduction
Endoscopic Submucosal Dissection (ESD) is an advanced endoscopic procedure used to remove early-stage gastrointestinal cancers. It allows for the en bloc resection of large or complex lesions from the stomach, esophagus, or colon. This minimally invasive technique provides accurate pathological assessment and reduces the need for open surgery.
Stepwise Approach in ESD Surgery
The ESD procedure begins with the insertion of a high-definition endoscope to carefully visualize the lesion. A special injection is given beneath the mucosa to lift the abnormal tissue away from the muscle layer. Using electrosurgical knives, the mucosal layer is incised, and submucosal dissection is performed to completely separate the lesion. The en bloc resection ensures that the entire tumor, along with a margin of healthy tissue, is removed in one piece. Endoscopic hemostasis devices are applied to manage bleeding during the procedure. This advanced method requires high technical expertise and provides curative outcomes for patients with early-stage gastrointestinal cancers.
Why It’s Done?
- To remove early-stage gastric, esophageal, or colorectal cancers
- To treat large or flat precancerous lesions not suitable for standard polypectomy
- To preserve organ function while avoiding open surgery
- To obtain accurate histological specimens for cancer staging
